form not generating

hi every one..

i am new to openkm, as per documents and videos i had implemented an workflow and assigned to user, and for that user assigned task is displayed but form is not generating! can any one help?

you're talking about form at the moment is starting process or a form in dashboard view ( there's a minimal difference in way how form declaration form is done ).
